چکیده

Editor, We read with interest the article by Niessen et al. (1) entitled “Irreversible Electroporation of a Hepatocellular Carcinoma Lesion Adjacent to a Transjugular Intrahepatic Portosystemic Shunt Stent Graft”. In the article, the authors detailed the use of irreversible electroporation in the treatment of a hepatocellular carcinoma (HCC) adjacent to a transjugular intrahepatic portosystemic shunt (TIPS). While we agree with the authors that a HCC located adjacent to a TIPS poses a significant treatment challenge, we do not agree with the view that “radiofrequency ablation (RFA) in the vicinity of the TIPS stent-graft was ruled out because of

Irreversible Electroporation of a Hepatocellular Carcinoma Lesion Adjacent to a Transjugular Intrahepatic Portosystemic Shunt Stent Graft

We report in a 65-year-old man hepatocellular carcinoma adjacent to a transjugular intrahepatic portosystemic shunt stent-graft which was successfully treated with irreversible electroporation (IRE). IRE is a new non-thermal tissue ablation technique which uses electrical pulses to induce cell necrosis by irreversible membrane poration. IRE proved to be more advantageous in the ablation of peri...

Transjugular Intrahepatic Portosystemic Shunt Occlusion Complicated with Biliary Fistula Successfully Treated with a Stent Graft: A Case Report

A 43-year-old man with liver cirrhosis received transjugular intrahepatic portosystemic shunt (TIPS) for the treatment of recurrent variceal bleeding and F3 esophageal varices. During routine follow up liver ultrasound examination, six months after the implantation, TIPS occlusion was suspected and TIPS revision was performed. During the revision, moderate to severe stenosis at the hepatic veno...

Transjugular intrahepatic portosystemic shunt for a wait list patient is not a contraindication for orthotopic liver transplant outcomes.

OBJECTIVES A transjugular intrahepatic portosystemic shunt for treating recurrent variceal bleeding or refractory ascites can be used as bridge therapy in patients awaiting a liver transplant. However, incorrect placement of the transjugular intrahepatic portosystemic shunt may complicate surgery during a liver transplant. This study sought to analyze a cohort of transplanted recipients to unde...

Treatment of chronic ascites due to right-sided heart failure by transjugular intrahepatic portosystemic shunt stent-implantation after orthotopic heart transplant.

Long-term outcome after orthotopic heart transplant is often critical in patients with long-persisting right-sided heart failure and consecutive ascites. Transjugular intrahepatic portosystemic stent shunt is an effective treatment for chronic ascites. However, a case of transjugular intrahepatic portosystemic stent shunt implantation after orthotopic heart transplant and left ventricular assis...

Recanalization of an Occluded Intrahepatic Portosystemic Covered Stent via the Percutaneous Transhepatic Approach

A 41-year-old woman with liver cirrhosis had recurrent portal hypertension and bleeding from esophageal varices due to complete occlusion of a previously inserted transjugular intrahepatic portosystemic shunt stent. Because recanalization of the stent by the transjugular approach was unsuccessful, ultrasound-guided entry to the splenic vein and portal vein was used. After catheter-directed intr...
